There’s no need to download, print, or hunt for anything here. When you call to book your first visit, our team will send your paperwork directly to you so you can complete it at home, at your own pace, without a clipboard in your lap and a waiting room full of people.

What We’ll Send You

Your new patient paperwork covers your dental and medical history, your current medications and allergies, your insurance information, and our practice policies. It takes most people about ten minutes.

Completing it in advance means your appointment starts on time and your full hour goes toward your exam and your conversation with the dentist — not toward paperwork.
